Below are the links to a series of Qualtrics Forms you’ll need for your programs and general RA shenanigans. Each form has its own purpose, and it’s important that you submit your forms well in advance of your event. Click on the title of each form to open it.
RA Program Form (click here to access this form)
- Used to request equipment for your programs
- See what’s available in the Equipment Calendar here before booking
- Due at least 1 week before your program
- Used to request a room booking on campus for a program
- This needs to happen 2 weeks before the program because the Central Booking Office needs time to process the request
If you’d like to buy something for your program and need to submit a Budget Request, please use the Budget Request form on eRez. Budget requests need to be submitted at least one week before you go shopping. If you shop before you hear back about your budget, you risk not getting reimbursed for your items.
Residence Life Cleaning and Event Form (click here to access this form)
This form is used to let Dexterra (caretaking services) know if there’s an exceptional mess in residence (i.e., vomit in a stairwell), or if you’re holding a program that might require extra cleaning (like extra garbage bins for a cooking program). You should not use this form to let Dexterra know you’ve used a cleaning kit. Cleaning kits are handled by our Maintenance team, and you can email shhs.maintenance@ubc.ca if there’s something missing or the wet vac is broken.
Front Desk: Printing Requests (click here to access this form)
These are to ask the Front Desk to print RA-related things for you. Please give them 2 days to process any requests. Please note that colour printing requests need to be approved by your RLM and may take a little longer to process. Colour printing is typically only approved for large-scale events and posters/decorations that will be used for the whole term/year.
Front Desk: Cash Box Request (click here to to access this form)
This is to ask the Front Desk to prep a cash box for you if you’re collecting donations or selling tickets for a program. Please give the Front Desk one week to prep your cash box.
PRC Shopping List (click here to access this form)
This is to let the PRAs know that a certain supply in the PRCs is low or that there’s something not normally stocked in the PRCs that you think would be useful to start supplying in the PRCs.
